A frame­work for health care pro­fes­sion­als of all dis­ci­plines to iden­ti­fy patients who have been diag­nosed with a seri­ous ill­ness, have ear­li­er con­ver­sa­tions about what is most impor­tant to them, and main­tain the integri­ty of the patients’ val­ues and beliefs as they go through their ill­ness jour­ney. The course includes an inter­preter tool (Seri­ous Ill­ness Con­ver­sa­tion Guide) acces­si­ble in 11 dif­fer­ent languages.
